Free Annual Basketball Camp – Terms and Conditions
Hosted in Pontiac, MI
Cost: Free | For Girls 2nd-8th grade and Boys 2nd-6th grade (including rising 7th graders)

1. Participation Requirements

   • All campers must have a signed registration form from a parent or guardian.

   • Campers must meet age eligibility and follow all rules and instructions.

2. Code of Conduct

   • Campers must be respectful, responsible, and refrain from bullying or unsafe behavior.

   • Disruptive conduct may result in immediate dismissal without notice.

3. Liability & Medical Consent

   • I understand the risks of physical activity and waive all liability from organizers and volunteers.

   • I authorize emergency medical treatment if I cannot be contacted in time.

4. Photo/Video Release

   • I give permission for my child to be photographed or recorded during camp.

   • These may be used for promotional purposes. (Optional — notify us if you do not consent.)

5. Weather & Cancellation

   • I understand that camp may be canceled or rescheduled due to weather or emergencies.

6. Cost-Free Guarantee

   • This camp is 100% free and involves no financial obligation.
